Required Forms:

The following forms are required for participation in marching band/color guard and for attendance at Band Camp. Please note the varying due dates.

• Band Camp Letter, Application, & Agreement - due in May
• Emergency Medical Form - due in May
• Physical Form - due in July at Pre-Camp
• Medical Summary (for Band Nurse) - due at Band Camp parent meeting

Optional Forms:

 If your child will be taking prescription or over the counter medications of any sort while at camp, you must fill out and return the from below that applies to your student. These forms should be turned in to Mr. Roemer or to the nurse at the Band Camp parent meeting in July.
